Description of Position
CCBC, a Behavioral Health Community Partner is hiring a full-time Intake Care Coordinator (ICC) to join our team. The ICC will be responsible for managing registration, intakes, engagement and overall assignment of members for CP care coordination services. The ICC is the first point of contact who talks directly to members, their families and providers to help determine social, medical and behavioral health care needs.
In addition, the ICC will provide care coordination to (40+) members, facilitate the development of a Care Planning Team and coordinate with team members to ensure implementation of the Patient-Centered Treatment Plan. The ICC will continue to work directly with the client, his/her primary care and behavioral health and social service providers to implement the Treatment Plan. The ICC is responsible for managing and directing members to the services they need.
Requirements
Must have good organizational skills, effective written, verbal, and computer skills. Must have experience navigating health care delivery systems, managed care and advocating for clients who are involved with behavioral health systems. Bachelor’s degree in related field or appropriate human services experience and/or language ability required.
