Description of Position
The Behavioral Health Community Partner (BHCP) Lead Registered Nurse assists in development, clinical coordination, and supervision of the BHCP program and its team members, including policy adherence and development, outcome data analyses and establishing community presence. Additionally, the Lead Registered Nurse completes Clinical Coordinator activities consisting of providing consultation, education, and support to engaged enrollees, team members and healthcare providers, as well as reviewing community assessments, care plan development, teaching health literacy and supporting care transitions. These duties may be completed in various settings, including community based or an inpatient facility, as well as telephonically or through other electronically based forms of communication, such as interactive applications.
Requirements
Must have active Massachusetts RN license. Experience in behavioral health and well as supervisory/program management experience preferred. Applicants must have a valid driver's license and reliable transportation.
