Description of Position
Elder Mobile Outreach Team (EMOT) is a multidisciplinary outreach team working with elders in the community to support their independence and overall quality of life. The team works to assist elders to remain in the community, live independently, and provides comprehensive support to address co-morbidities such as medical and mental health needs.
Work as a part of a multidisciplinary team to conduct clinical assessment, direct services, and complete documentation for a group of elders served by the EMOT. Flexible schedule – 20-30 hours. Competitive salary and excellent benefits package.
Responsibilities:
- Process referrals for intake/admission into program;
- Conduct and complete psychosocial assessments;
- Develop and write treatment plan in conjunction with elders;
- Present assessments at multidisciplinary team meetings;
- Conduct at-home visits with elders in the community;
- Complete documentation in a timely fashion, including incident reporting;
Requirements
Master’s degree in human services related discipline. Knowledge of Core-Competencies as identified by the employee’s assigned department; Must have a valid driver’s license in the state of residence and submit a copy of satisfactory record of driving from the state of residence of registry of motor vehicles, as determined by the department vice president; Must complete MA and ADP Criminal Offense Record Information (CORI) process; Must have computer literacy as related to the requirements of the position.
